    Instructions for Registration

   1.   Fill out registration form and click 'submit'

   2.   Choose payment:   online - below
          or send a check to:  Multi-Arts -PO Box 108 Hadley, MA 01035
​​3.   Send the following documents: 
​        we need them in order to complete registration
           1.   Certification of up-to-date immunization 
            2.   Most recent physical exam
            3.    Download, sign and return the Waiver Agreement
            4.   Email PDF or a photo of the signed waiver to:

Multi-Arts membership is $45 per family
and covers all registration fees of the summer camp,
​winter and spring programs, ​and workshops from
 January 1st - December 31st
